Cardiovascular Rehabilitation through Physical Therapy
PT Plus is pleased to offer a variety of exercise services for patients suffering from and managing any of the following medical conditions: congestive heart failure, cardiopulmonary issues such as COPD, myocardial infarction, CABG, heart valve repair/replacement, stable angina, peripheral artery disease, and diabetes mellitus.
Phase III Cardiac Rehab
- Non-monitored / supervised exercise
- Maintenance level exercise and education program
- For patients with prior cardiac history, or significant cardiac risk factors (hypertension, high cholesterol, diabetes, stable angina)
- Closer supervision and physician correspondence
Phase IV Cardiac Rehab
- Non-monitored / supervised exercise
- Maintenance level exercise program for those who have completed a Phase III program and have been allowed by their physicians to participate in a less-monitored program
PT Plus Lifestyles Program
- For new exercisers with significant health risk factors
- Medical assessment and customized exercise program
- Learn how to use exercise and lifestyle changes to manage blood sugar, body weight, blood pressure, cholesterol
- Wellness lecture series
- Group exercise classes
Our licensed physical therapists and staff can track patients' blood pressure, SPO 2, and heart rate, and do not monitor with EKG. There is an AED on-site, and all staff are certified in CPR with an AED. Since insurance carriers do not reimburse for these services, monthly rates are $49 for an individual.
